COMPLETED DOCUMENTS NEEDED:
- Completed Dispatch Agreement Form - Note this agreement does NOT have any length of time limits or cancellation issues. We require this agreement to meet FMSCA regulations.
- Completed Dispatch Authorization Form - This form allows us to represent you and your company in securing freight for you.
- Completed Company Profile - Please be detailed. The more information the better we can handle you needs.
- Equipment Data Sheet - If you have more than 1 truck, please use a form for each truck.
DOCUMENTATION PROVIDED BY YOU:
- Copy of your Authority
- Completed W9 Form - A blank one is included in our set up packet.
- Insurance Certificate - We need to be listed Certificate Holder. Details are listed in the set-up packet.
- List of Business References - If you are already established with any.
